Clerk, HIM - Full Time - 1/18/19

Bargaining Unit:  Non Licensed                            Rate of Pay:  $16.64 / HR + DOE

SUMMARY:  Responsible for processing medical records for Tahoe Forest Health System.  Performs clerical functions for HIM Department.


Processes Inpatient; Outpatient, and Ancillary accounts; including Lab; X-ray; all clinics; Therapies; Ambulatory Surgery; and the Emergency Room patient charts. 

Reviews and analyzes inpatient, emergency room, ambulatory surgery accounts for completeness.

Performs qualitative and quantitative assessments of scanned documentation in the Health Information System (HIS).

Provides information and assistance to physicians and staff as it pertains to record completion. 

Performs weekly mailings of notices to follow-up with practitioners regarding incomplete records.

Uses computer to process, store, and retrieve health information in the HIS.

Answers telephones and assist callers, or routes call to appropriate person.

Retrieves patient medical records using the terminal digit numeric filing system for inpatient, emergency room records, and ambulatory surgery records.

Interfiles loose material into the patient record.

Retrieves patient information of all admission types per request, utilizing the HIS and filing system.

Processes requests for release of information in accordance with HIPAA and Privacy law standards for all clinics and outpatient departments, and billing requests. 

Uses release of information standards for inpatient and legal requests.

Reviews and analyzes potential duplicate medical records, and corrects those valid duplicates.

Scans hospital records, patient information, and other hospital documents to support departmental operations.

Reviews, prepares, and scans emergency room records retrospectively and concurrently following written procedures.

Faxes documents upon request, and as applicable.

Assists physicians and other staff members with transcribed reports.

Travels to offsite TFHS facilities to process work, when required.

Maintains and utilizes a variety of health record indexes.

Demonstrates System Values in performance and behavior.

Complies with System policies and procedures.

Other duties as may be assigned.

EDUCATION AND EXPERIENCE:  High school diploma or general education degree (GED); One to two years related experience and/or training; or equivalent combination of education and experience.

LICENSES, CERTIFICATIONS:  Required:  Valid Driver’s License

Preferred:  Certified Coding Specialist (C.C.S.) certification.

OTHER EXPERIENCE/QUALIFICATIONS:  Knowledge of release of information standards for inpatient and legal requests
